Tummy Tuck (abdominoplasty)
A tummy tuck (abdominoplasty) surgically removes excess skin and fat from the abdomen and tightens separated muscles that often occur after pregnancy. Though a tummy tuck offers the benefit of an improved abdominal contour, scarring is an inevitable result. Unlike liposuction, which produces minimal scarring, abdominoplasty actually removes skin, creating much larger marks. The extent of scarring will be determined by the nature of the issues being addressed. Those with a considerable amount of skin to be removed will benefit from improved contour, but a hip to hip and as well as a scar around the belly button will also result. For patients with less extensive issues, such as limited skin excess and muscle laxity which only occurs below the belly button, much smaller incisions are needed, resulting in less extensive scarring. This procedure is known as a "mini-abdominoplasty." Scars from many mini-abdominoplasties are similar to the extent of scarring incurred during a hysterectomy or C-section. Though tummy tuck incisions are placed to be hidden as much as possible, below underwear and bathing suit lines, potential patients must carefully weigh the aesthetic benefits of the surgery against the scarring issues when deciding whether to undergo the procedure. The Consultation During consultation a discussion of medical history and a patient's aesthetic goals occurs. An examination evaluates issues such as body contour, degree of skin and fat to be removed, muscle and skin tone and skeletal changes. Any patient more than 50% above their ideal body weight should consult a weight loss specialist before undergoing cosmetic surgery. Patients should also postpone tummy tuck surgery until they have completed child-bearing. It is also essential smokers cease smoking for at least one month before to surgery and one month following. Smoking can cause significant healing issues in tummy tuck patients. Every individual has specific goals and unique anatomical considerations. The detailed consultation will address these issues for individualized results. The Surgery This procedure should always be performed fully accredited facility by a board certified plastic surgeon. Anesthesiologists should also be board certified for optimal safety and comfort. As a plastic surgeon, I like to create an incision plan with the patient the morning of the surgery, taking into account preferred underwear and bathing suit styles. Fat contouring areas are marked before surgery as well. During the procedure, excess skin is removed as well as fat from the abdominal wall. Muscle tightening will also be performed. An extra benefit of abdominoplasty is a reduction of stretch marks, though complete stretch mark removal is should not be expected. Though the procedure can significantly decrease abdominal skin and excess fat and tighten tummy muscles as well, an ideal “hourglass” figure will not be surgically achieved. The Recovery While the surgery is performed under general anesthesia, Dr. Orringer will use local anesthesia during the procedure, as well, to assist with pain control early after surgery. Patients usually spend 1-2 nights at Serenity, a warm, comfortable facility geared for the care of the post-operative plastic surgery patient. Pain is usually well-controlled with oral analgesics. However, stronger pain medication may be given through the IV, if necessary. Ambulation is encouraged the night of surgery. Pneumatic pumps will be used on the legs in an effort to prevent blood clots until you are ambulatory. Dr. Orringer will see you the day following surgery. Most patients can return to light work activity by 10-14 days. Driving should not be done until oral narcotics are no longer required for pain control. Exercise activity can progress, as directed by Dr. Orringer. Normal exercise activity is usually permitted at 6 weeks. The tissues feel firmer than normal for at least 1-2 months. Most swelling has subsided by 3 months. Scars will typically continue to fade for 2 years. What are some of the Particular Concerns regarding Tummy Tuck surgery? While abdominoplasty offers a high rate of patient satisfaction, certain issues must be considered carefully before proceeding. Because of incisions used in skin removal, permanent scarring will result. While it does tend to lighten during the first 2 years, the scar is permanent and in some cases, long. With full abdominoplasty, a scar around the navel is present, as well. A patient must be willing to accept this as a trade-off for potentially improved contour. It should also be noted that though stretch marks are often reduced, but some still remain after the procedure. In addition, marked narrowing of the waist and an ideal "hourglass figure" are not realistic expectations of the procedure. Loss of sensation is also a risk, and though it is generally temporary, permanent loss may also occur. Most sensation loss occurs in the belly button area, near the mid-line. Though abdominoplasty offers significant potential benefit to patients who undergo the procedure, it is also a major procedure that carries with risk as well. Making the decision Like all surgical procedures, abdominoplasty carries inherent benefits, risks and limitations. As a plastic surgeon, I feel careful pre-operative consultation is essential in attaining optimal results from this or any procedure. Surgeons should make potential patients feel comfortable asking questions and expressing concerns and goals so they can make informed choices about the procedures they undergo.
